Overview
The purpose of the Transitions program is to bridge the gap from adolescence to adulthood, providing service linkage, therapy, support, and empowering young people to gain skills to achieve independence.
The program focuses on the following areas as needed:
- Criminal justice system involvement
- Educational status
- Employment status
- Living situation
- Mental health issues
- Substance abuse issues
- Wellness
Individuals will be supported in pursuing wellness in all aspects of their lives:
- Attaining and maintaining employment
- Ensuring appropriate living arrangements
- Graduating from High School and furthering educational achievement
Eligibility
This program is open to individuals ages 18 to 25 who meet the following qualifications:
- Exhibit impaired functioning in their home, school, or community, limited family resources, or are transitioning from foster care or another structured living situation
- Received Human Services for at least 2 years
- Residents of Venango County and meet medical necessity as determined by funding sources
